Minimum Qualifications

Seven years of experience in developing, managing, or evaluating health or social service programs; or in public health, public relations, social services, health services, health regulation, or administrative services; OR

Six years of full-time work experience in any field plus four years of experience in developing, managing, or evaluating health or social service programs; or in public health, public relations, social services, health services, health regulation, or administrative services; OR

A bachelor’s degree plus four years of experience in developing, managing, or evaluating health or social service programs; or in public health, public relations, social services, health services, health regulation, or administrative services; OR

An advanced degree or a Juris Doctorate plus three years of experience in developing, managing, or evaluating health or social service programs; or in public health, public relations, social services, health services, health regulation, or administrative services.

EXPERIENCE SUBSTITUTION:
Every 30 semester hours earned from an accredited college or university will be credited as one year of experience towards the six years of full-time work experience in any field. The maximum substitution allowed is 120 semester hours which substitutes for a maximum of four years of experience in any field.

Job Duties and Other Information

Job Duties:

  • Implements and oversees the Operational Protocol to support statewide transitions from institutional settings to home and community-based services using NOW, ROW, and CC waivers
  • Ensures compliance with OCDD and Medicaid waiver policies, adapting to evolving regulations and program requirements;
  • Monitors program performance by analyzing data indicators, tracking transition goals, and developing budget projections and reports;
  • Identifies and addresses program challenges proactively, implementing solutions to meet grant benchmarks and improve outcomes;
  • Coordinates and promotes pre- and post-move support, including housing assistance, family training, workforce development, and transition coordination services;
  • Represents the My Place program at statewide councils, workgroups, and professional meetings, providing education and program updates;
  • Prepares and delivers comprehensive reports and data to leadership, Medicaid, CMS, and stakeholders, including budgets, quality metrics, and program outcomes.
